Speech Unlimited provides speech and language therapy in a child's most natural environment. This includes homes, daycares, and schools. This allows the session to be as effective and functional as possible. Our therapy is play based with specific goals targeted. Our commitment is to provide the highest quality speech therapy in a fun learning environment for children. We currently provide speech therapy in the following counties: Cabarrus, Mecklenburg. Iredell and parts of Rowan.
Early identification and intervention leads to early success with speech and language problems. Early intervention has been shown to significantly improve communication skills thus decreasing behavior problems and a child's frustration. A comprehensive evaluation will determine the child's risk factors and make recommendations for the appropriate course of treatment. A copy of your child's evaluation will be provided for your records.
Parents, caregivers, and family members are recognized and supported as valued partners in the intervention process. Scheduling is flexible and will be determined with the family and therapist. We offer the following

Speech Unlimited, Inc. is required by law to protect the privacy of your personal health information, provide this notice about our information practices and follow the information practices that are described herein. We protect and establish appropriate safeguards to prevent, the unauthorized disclosure of PHI in accordance with state and federal law, including any regulations governing the security of PHI and the transmission, storage or maintenance of electronic data that contains PHI. Speech Unlimited does not use or disclose PHI in any manner inconsistent with the use and disclosure restrictions placed on the Covered Entity by HIPAA.
USES AND DISCLOSURES OF HEALTH INFORMATION:
Speech Unlimited uses your personal health information primarily for treatment; obtaining payment for treatment; conducting internal administrative activities and evaluating the quality of care that we provide. For example, Speech Unlimited may use your personal health information to contact you to provide appointment reminders, or information about treatment alternatives or other health related benefits that could be of interest to you. Access to confidential information is allowed only to staff that have an authorized business requirement to view the Confidential Information. You can ask us to contact you in a specific way (for example, home or office phone) or to send mail to a different address. We will say “yes” to all reasonable requests. Speech Unlimited may also use or disclose your personal health information without prior authorization for public health purposes, for auditing purposes, treating you, billing for your services, research studies, to comply with the law, help with public health and safety issues, respond to lawsuits or legal actions, and for emergencies. We provide information when required by law. In any other situation, Speech Unlimited’s policy is to obtain your written authorization before disclosing your personal health information. If you provide us with a written authorization to release your information for any reason, you may stop future disclosures at any time. We shall not use, publish, transfer, sell or otherwise disclose any Confidential Information gained by reason that is not directly connected with the performance of services contemplated hereunder except as provided by law; or in the case of Personal Information, with the prior written consent of the person or personal representative of the person who is subject of the Personal Information. Speech Unlimited may change its policy at any time. When changes are made, a new Notice of Information Practices will be provided to you on your next visit. You may also request an updated copy of our Notice of Information Practices at any time.
PATIENT’S INDIVIDUAL RIGHTS:
You have the right to review or obtain a copy of your personal health information at any time. You can request a copy of your electronic medical information in electronic format. Speech Unlimited will have 30 days to produce this information. We may charge a reasonable, cost-based fee for this. You have the right to request that we correct any inaccurate or incomplete information in your records. You have the right to choose someone to act for you on your behalf. If you have given someone medical power of attorney or if someone is you legal guardian, that person can exercise your rights and make choices about your health information. We will make sure the person has the authority and can act for you before we take any action. You can choose who you would like for us to share information with such as your family, close friends or others involved in your care. You also have the right to request a list of instances where we have disclosed your personal health information for reasons other than treatment, payment or other related administrative purposes. You may also request, in writing, that we not use or disclose your personal health information for treatment, payment and administrative purposes except when specifically authorized by you, when required by law or in emergency circumstances. For patients that pay for services personally and in full, you can require that Speech Unlimited not share your information about the treatment with your healthcare plan unless a law requires us to share that information. Speech Unlimited will consider all such requests on a case by case basis, but the practice is not legally required to accept them. Speech Unlimited will notify covered entity of any potential breach of security or privacy. For instance, where the PHI is subpoenaed, copied or removed by anyone except an authorized representative of Speech Unlimited. We will not contact you for any fundraising efforts.
